The story is enticingâ€"a young person or previously responsible parent goes astray and succumbs to the ravages of alcohol or drugs, is confronted by caring friends and family, and in just one episode, realizes the error of their ways and turns their life around, often through a miraculous yet accessible treatment program. But is the reality really as simple as reality TV would have us believe? Although shows featuring real-life interventions are helpful in giving hope to addicts and their families, and showing that change is possible, they don’t necessarily reflect an accurate view of reality, either for the individuals involved or how an intervention could turn out for those considering an intervention for someone they care about. Do Interventions Work? The term intervention can be confusing because it can be used to refer to the various therapeutic approaches used to treat addiction, many of which are evidence-based  and effective. These include motivational interviewing, cognitive behavioral therapy, and couples therapy. These evidence-based  treatments  and several others, typically take time and commitment on the part of the addicted person but are generally helpful. What we are discussing in this article is not a treatment per se, but rather, a planned attempt by a group of people to persuade someone they are in a relationship with to either quit alcohol or drugs on their own or to seek treatment. This process may be led and guided by an interventionist who is hired by the family or group. An intervention may not be done in an aggressive way,  although sometimes it is accompanied by expression of anger about past wrongdoings of the addicted person, and threats to discontinue or seriously limit the relationship, but it is always in some way confrontationalâ€"even if the confrontation is presented as an invitation, which generally is not effective in overcoming addiction. From a professional standpoint, interventions cannot be recommended, simply because there is not enough research available to support their effectiveness. That doesn’t mean they can’t be effective; it just means that the studies we need in order to “prove” the effectiveness of interventions has not yet been carried out. While a few studies were conducted on the effectiveness of interventions in getting people into treatment during the late 20th century, they typically showed that family members chose not to follow through on confronting their family members. One study showed that when they did follow through, they were able to get their family member into treatment, but in the end, this was a very small number of people, and the outcome of therapy was not reported. Bear in mind that all therapies, no matter how effective, were at one time unproven, went through experimental stages and refinements, were funded for research, and finally, enough studies were conducted showing their effectiveness that they became accepted the practice. But just because they are accepted practice doesn’t mean they are guaranteed to work for everyone. From an anecdotal perspective, interventions have mixed reviews. Some clinicians have had the experience of working with people whose families have conducted interventions that have been helpful in persuading their loved one to get help. Others have had much more negative reviews, in which the intervention was poorly conducted or the addicted individual was not in a place to hear the feedback, and it caused an even greater problem for the addicted individual and an even greater rift in their family. So how do you know whether an intervention  is going to help the person you care about? What seems to be the crucial factor is the experience and skill of the interventionist leading the interventionâ€"perhaps this is why interventions appear so successful on TV shows with large budgets, the best-trained experts, and teams of professionals providing backup support to the individual and family. But in the real world, we have no current systems for evaluating the credentials of interventionists, nor for providing backup if needed, so there is very little information on which to base your decision. Why Would People Use Interventions If They Dont Work? Interventions for addiction are big business, especially in the United States, where they are often portrayed on TV. In desperation, families of people with addictions pour their life savings into interventions, hoping to save a loved one who seems to no longer see reason. Yet the basis of interventions is more about hype than about true recoveryâ€"reality TV is not in the healthcare business, it is in the entertainment business, and the drama of a carefully orchestrated and edited video story of confrontation and redemption is very appealing to the modern American audience. Part of why interventions are so appealing, and also, so unlikely to be effective, is that they offer the dream of a simple solution to an incredibly complex situation. We know from decades of research that people do not become addicted purely by nature or nurture, but a complex interplay between the two. Typically, someone with an addiction is struggling with underlying issues that they may not even be aware of, issues that an interventionist or well-meaning family member are even less likely to be aware of. Although a proportion of people are able to overcome severe addictions on their own, it takes great determination and access to alternative ways of coping to do this. For many others, overcoming addiction requires treatment, and it often takes many attempts to completely quit alcohol and drugs. Of course, this does not mean that people are never helped by an intervention. The process of becoming aware that your behavior is harming yourself, and those around you, is an important step towards recovery, and the first step through the stages of change from pre-contemplation to contemplation. However, confronting someone with an addiction is a very risky approach, and can just as easily backfire, making the addicted person feel attacked, alienated, and misunderstood instead of feeling supported. In these cases, an intervention can even worsen an addiction, causing the person to seek comfort in alcohol and drugs, and to seek out the company of those who understandâ€"drinking and drugging buddies and drug dealers. The Different Stages of How to Overcome Addiction Factors to Consider in Choosing an Interventionist If, after considering the lack of research support, you still feel an intervention might be right for your loved one, here are some common-sense considerationsâ€"NOT based on medical fact or researchâ€"to think about in making the decision to employ an interventionist: Do you know anyone who was helped by the interventionist? If so, were their problems similar to those of the person you care about? Were their characteristics (such as age, culture, and how long they have had the addiction) similar? A personal recommendation from someone dealing with similar issues is often a way that people make decisions about therapists.Talk to the interventionist. Did you develop a good rapport? Do they seem to understand the problems you are describing? Do you get a good “gut feeling” from this person?What is the financial investment? How much do you stand to lose if it doesn’t work out? Could these funds be used on a different program that offers more credibility? Remember that although many people working in the addictions field know what they are doing and genuinely want to help their clients, there are others out there who just want your money, and will prey on the desperation felt by loved ones who are looking for a miracle cure. There are no miracle cures, and overcoming addiction is hard work, especially for the addicts themselves. Alternatives to an Intervention The best approach to treating an addiction depends on a great many factors, including the substance being used, how severe the addiction is, the addicted persons attitude towards treatment and quitting or cutting down, and whether they have concurrent mental and/or physical health problems. A physician is often in the best position to discuss this with the person with the addiction, although many physicians do not have a great deal of training or expertise in addiction, and some are not comfortable working with people with addictions. The American Board of Addiction Medicine provides specialized training and certification for physicians, and so an ABAM certified physician is a great person to help people with an addiction find the right treatment, as well as provide a great deal of treatment themselves. You might also want to consider finding out about other treatment options for your loved one: DetoxMotivational interviewingResidential treatmentSelf-help groups, such as SMART recovery and Sober24 Communicating With Your Loved One Communication with your loved one is important in building and maintaining the trust that is required to support them in recovery. Often, trust in relationships can be badly damaged during an addiction, as the addicted person will often feel it necessary to keep their addictive behavior secret from their loved ones, often believing this is for their own good. Then, when the loved one discovers the addictive behavior or is faced with some of the negative consequences of the addiction, they typically feel betrayed and angry. You may not be able to persuade or bully your loved one into treatment, and in fact, trying to do so may actually make both their addiction and your relationship worse. On the other hand, honest and clear communication is typically very helpful. Setting boundaries around what kinds of behavior cross the line and are unacceptable, and making it clear that you have your own life and needs are extremely important in relationships with people who have addictions. The motive (F – Ab – F) is said to be derived from a saying of Brahms’ close friend Joseph Joachim, â€Å"Free, but lonely† or â€Å"Frei aber einsam†. Taking the first three letters of each word you get F- A- E. This short motive Brahms, Schumann and Dietrich used to compose a violin sonata in honor of their friend. Brahms later declared himself â€Å"Free, but happy† or â€Å"Frei aber froh†. This gives us the motive F – A – F. Why, then, does Brahms choose to use an Ab instead of natural A? Probably because the Ab gives his harmony a non–traditional sound. Instead of the regular chord progression he uses an F major chord to a diminished 7th. This should resolve to a C major, but instead it goes back to F major. It then slides to an F minor chord, and then to an unexpected Db minor chord before another diminished 7th, finally resolving to C major, as it should. This motive is heard throughout the symphony, whether quite obvious as in the first three bars, or in the bass line as in bars 4-6. This unusual chord progression also explains why the last movement is in F minor, instead of the expected F major. The final movement of this piece leads you to think of dark, dramatic struggle due to the F minor. Rather than a tragic ending, he lets the clouds part and brings back the motive of F – Ab – F, and finishes his symphony as it began. The finale is lyrical, passionate and rich in melody. Culture is beliefs and values that are shared and influences effectiveness, they interact with formal structure to get behavioral norm. It is shared basic assumptions that are learned when solving problems of internal integration and external adaptation. There has been evolution of pattern of values, rites, practices, rituals and symbols. Culture is pattern of activities of human beings and symbolic structures that show significance of these activities. Characteristics of culture Culture is learned where its values can be transferred into institutions by means of social interaction and exposure to mass media. This exposure is very important because it reaches everybody and creates awareness about values and beliefs that are shared and no one can lack access to information. A purposeful culture offers order and gives direction and guides in solving behavior of human being because there is a given direction to be followed by every one and proper means of solving human behavior. The beliefs and practice must be common to the society because culture is shared. Members of the same society must have the same culture and share in the beliefs and practices that are set by the society. Culture is cumulative because, it is passed from one generation to another and each new generation is aware of the culture it is supposed to follow. This makes culture have a very strong historical and traditional relevance. (Barry, 2002 pp. 11-17) Culture is dynamic. In case of any change in society, it is able to adapt quickly. This is very important because the world is dynamic and changes can not be avoided at any given time and most of times changes are for the better due to innovations and changes in technology. Therefore, we should be careful on what we experience, see with our own eyes or hear because it can affect our beliefs either positively or negatively. Culture considers how members perceive things but not whether they like it or not. Sources of culture Culture is transmitted from one person to another through stories containing narrative of events and things like breaking of rules, reaction of past mistakes where one is disciplined after a mistake is done and the punishment available to wrongdoers. The stories are true but are not meant to link past events with present events but only provide an explanation of the performance of events in order to justify practices that are used currently. Culture can come from rituals which are sequence of activities that repeat themselves in order to reinforce key values. The important goals of the firm, ceremonies for recognition and awards and annual picnics are all considered as rituals. In higher learning institutions, the way academic graduates dress and academic procession are the rituals that reinforce great value and beliefs. Material symbols can reflect the seriousness of organization and the quality of product and service which reflect organization personality. The facilities layout of offices and buildings and the way of dressing of employees and the cars driven by the superiors are material symbols. Some places have large well organized offices for their staff and allow staff to be members of clubs and reserve places where managers park their vehicles. Symbols create corporate identity by making workers feel special although those who are not recognized may have negative effect. (Castells, 1999 pp. 15-20) Language can be used to identify members who belong to a particular culture. By learning the language, people show that they accept the language and culture and are ready top preserve it. In computer companies, there is unique language that is used to describe equipments and key personnel. New employee require more time to learn the language and coding system and once everyone learns the language, it is used as a key denominator that unites members to given culture. Culture is identified with civilization and contrasted with nature. Some countries are taken to be more civilized than others and some people are more cultured compared to others. Social paradigm and culture refers to specific society and a specified time. This means that, there is change from time to time and from place to place. Therefore, there is no theory to determine relationship that exists between culture and economic or political elements in a society. The starting point is rejecting versions of crude economists and sophisticated versions that determine elements that are dominant in social formation. Elements which are dominant in social formation can not be determined by economic base but are determined only by creative act, that is, whether the outcome is of social praxis of social individual’s activities. Theocratic society’s dominant element was cultural and society existed as socialism. In market economies, after introduction of new system in commercial society during industrial revolution, dominant elements in markets economies were also economic. Means of production was controlled by private ownership which led to economy which was socially controlled and market played major role in market economies due to fundamental self regulating systems and are solved by price mechanism rather than by use of social decisions. Interdependence is the relationship between elements with economic element dominating. There are differences that exist between social paradigm and culture because the scope of culture is great and expresses ideas and values that are not necessarily consistent with dominant institutions and characterizes market economy arts and literature where there is significant freedom given to writers and artists to be able to express their own views freely. But in dominant social paradigm the values and beliefs in market society must be consistent with economic elements where economic institutions that determine dominant elites are economic elites who control production. Society can not be able to reproduce itself without dominant beliefs and ideas which are consistent with institutional framework. For example, the social paradigm which is dominant is supposed to be dominant with political elements and political institutions which determine party bureaucracy. The future democratic society corresponds with democratic institutions to ensure there are no formal elites in the society though democracies can not function well without informal elites. (Baudrillard, 1988 pp. 44-47) Mass media produces reality and does not fake things. Mass media does not manipulate democracy because it is democracy which is faked and not the picture of mass media and this shows the reality of current democracy and defines political reality provided by mass media. Furthermore, there can be distorted picture provided by mass media when reported about what causes crisis conflict between different sections of elites. This is where they show the picture which the sections that control them may want to reproduce. For example the media in Anglo-American distorted meaning of Iraqi criminal bombardment in 1998. During the Gulf war, there was distortion of what caused the conflict such as who would be in control of oil in the world with legumes of peace loving between the north and south regimes. Under circumstances like this, mass media is the only one that can tell the truth of the reality when there is division between elites on the way they take political reality. Mass media is very significant whether it is owned by few people or many in order to help in struggle aiming at social change. For example, division among European elites to join monetary union have led to great discussion by the media on meaning of European integration in Britain where there was split of elites. Similar divisions existed between European and Anglo-American over the Gulf crime which was criminal role of supporters of bombardments. About economic reality, there is accurate picture provided by mass media on economic reality today.
